Support Broker

Responsibilities will include but are not limited to the following:
Assist the individual/ family in completing the startup process according to OPWDD requirements and program procedure.
Assist the individual and family in maintaining the self-direction plan/budget including monitoring expenditures to ensure that expenditures do not exceed the self-direction budget.
Assist the individual in building their Circle of Support and facilitate Circle of Support meetings.
Assist the individual in developing a comprehensive Person Centered Plan with the involvement of the Care Manager and Circle of Support
Ensure completion and regular updating of habilitation plans for self-hired Community Habilitation and Supported Employment (SEMP) staff if these services are chosen.
Assist the individual in properly documenting self-directed services according to Medicaid and OPWDD regulations and policy including timesheets, service documentation, mileage and expense reimbursement forms, and all other required documentation.
Assist individuals as specified by each broker contract of services with such things as annual meetings, plan management, revisions, amendments, Circle of Support facilitation and provide support around employee issues.
Ensure that Circle of Support meetings occur at least two times per year. The participant and the support broker must be face-to-face (in person) for these meetings.
Attends and participates in individual conferences, staff meetings, training sessions and professional development opportunities as required.
Complete annual professional development as per OPWDD regulations.
Maintain necessary records and documentation for each consumer served.
Act in a manner consistent with Central New York Quest’s Mission.
Other duties as assigned by the supervisor.
